Temporary Guardianship Form - Updated

A Temporary Guardianship Form is a legal document that gives a person temporary guardianship of a child. This form is used when the child's parents are unable to care for the child, or when the child needs to be in the care of someone other than the parent for a short period of time. The form must be signed by the child's parent or legal guardian, and must be notarised.

There are many reasons why someone might need a Temporary Guardianship Form. For example, if a parent is going to be out of town for an extended period of time and needs someone to care for their child, they may use a Temporary Guardianship Form to appoint a temporary guardian. Additionally, if a parent is hospitalized and unable to care for their child, a Temporary Guardianship Form may be used to appoint a temporary guardian.
