This is a sample of Contact US Form generally used in websites and blogs. This provides both the web owner and client an easy way to exchange information. This form is mostly used to gather information from people visiting a website or getting feedback from them. Contact Us Form is a simple form which gets the name, address, phone number, state, city and comment/query from the user and saves it in the server of the website.
